"I don’t know, but let’s up your pain meds and see.”
“But my pain is real! It’s right here!” Those statements from well-intended practitioners make you feel invalidated, gaslit, and thoroughly dismissed.
Yes. Your pain is real. All pain is real. It is felt in your body. It is physical. But, it is also physiological, emotional, and psychological. Pain is an experience of being human. Pain is designed to protect us from damage and danger. But pain is not supposed to kick around forever.
What about these messages?
“You have a disk bulge.” “Your ultrasound shows a large rotator cuff tear.” “You have the back of an 80 year old.”
And my personal nemesis,
“Your knee is bone on bone.”
These messages leave you with desperation – it's surgery or bust. And bust means having to live with the pain. Not an attractive option at all.
What most people don’t realize is that pain has a very poor relationship with tissue damage. There are many people with significant damage and no pain. There are others with no damage and debilitating pain. The likelihood is that if you’re not a young child, you’re somewhere in the middle with respect to tissue damage. There are many research studies showing degenerative changes like those above in people with no pain. Zero pain, but scans that show damage.
Consider this question: Is it possible that the damage was already there before the pain? If there’s even a possibility of this being true, that damage more often than not did not cause the pain. This realization is often the biggest pivot that someone can make in their recovery journey.
“But how do I know?” “What if there IS no damage?”
“What if the damage DOES cause pain?”
Let me tell you a story.
A client of mine, a man in his late fifties who ran his own construction business, came to me after almost two years of low back pain that had already outlasted two rounds of physiotherapy, months and months of chiropractic, and an MRI that "showed nothing serious." He'd stopped golfing. He'd stopped carrying his grandkids. He was convinced his back was somehow more fragile than it used to be.
It wasn't. What had happened was that his nervous system had learned, over two years of guarding, bracing, and fearing the next flare, to sound the alarm at movements that were never actually dangerous. The tissue wasn't the problem anymore. The alarm was.

Here’s another real life story:
A new mother of twins in her late twenties had tweaked her shoulder lifting a car seat. The pain was so severe she was unable to care for her young babies. She had the typical course of management. She started with her doctor who gave her pain killers and prescribed an ultrasound and physiotherapy. The ultrasound showed a rotator cuff tear, which the physio concurred with as being the problem. So off to a specialist who wanted to do surgery. But she couldn’t. She needed to be available to her kids.
The problem continued for over a year, and eventually the specialist determined that surgery was no longer an option. She was still in pain, barely able to do anything with her kids, barely able to make meals. The pain showed up every single time she lifted her arm. She was under the belief that she was permanently damaged. She was devastated and at the literal end of her rope.

We worked together to discover the true reason for her pain – it wasn’t the rotator cuff, which had long healed.
